1. Review the Application Guide
The first step in the application process, before completing the application, is to review the application guide. The application guide was developed to provide applicants with a better understanding of what to expect and how to prepare for the application and verification processes. The guide outlines all of the information and supporting documentation that will be requested in the application. The guide will be reviewed and updated as necessary, and the most up-to-date version will remain available on this webpage.
2. Complete the Application
The application is available to complete online in Qualtrics. Only 5 applications will be accepted per quarter on a first-come, first-served basis. To request a custom application link, email pedready@jax.ufl.edu. If all application spots are filled, your agency will be placed on a waitlist. Once the applicant receives their custom application link, they will have 30 business days to submit the application. Applicants will have the opportunity to provide and submit all requested information and supporting documentation in the online application.
Once the application is submitted, applicants will receive an email confirmation acknowledging successful receipt of the application. Application documents will undergo a confidential review by the Florida EMSC Project Director, Program Manager, and members of the Florida EMSC Advisory Committee or EMS Pediatric Readiness Workgroup. If further information is requested, the agency will be contacted by the Florida EMSC State Partnership Program Staff.

3. Participate in a Verification Visit
If the applicant meets the recognition requirements, a brief virtual, hybrid, or on-site verification visit will be scheduled. Applicants should expect a decision and recognition documents at the end of their verification visit.
4. Receive Recognition Status
Recognition: Agencies that are approved for recognition will receive an email notification, an official letter, a certificate, vehicle decals, and a sample media package. By applying for recognition, successful applicants approve their organization name and general information to be posted in program documents and on the Florida EMSC website. Approved agencies will be recognized and congratulated at the subsequent Florida EMS for Children Advisory Committee meeting. The recognition documents will be valid for three (3) years from the date of recognition.
Denial: If the application is incomplete or the information provided does not meet the criteria for the tier the applicant applied to, the individual who submitted the application will receive an email notification including the reason(s) for denial. Subsequently, the agency will be given the opportunity to work on application deficiencies and resubmit. The names of denied applicants will not be posted or shared in a public forum.
